I hereby grant my consent to the processing of personal data and information on the processing of personal data, as the data subject, to the company Celpap Machinery, s.r.o., with registered office at Politických vězňů 10, 110 00 Prague 1, IČ 60775564, registered in the commercial register maintained by the Municipal Court in Prague, section C , insert 96823, as the administrator of personal data (hereinafter referred to as the "administrator") in accordance with Article 6, paragraph 1 letter a) and Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons in connection with the processing of personal data and on the free movement of such data and on the repeal of Directive 95/46/EC (general regulation on personal data protection) (hereinafter referred to as the "GDPR regulation") free and unequivocal consent to the processing of my personal data (hereinafter referred to as "consent"), under the conditions set out below:

The data subject's personal data will be processed based on the administrator's legitimate interest for the purpose of direct marketing.

The reason for the provision of personal data by the data subject is the data subject's interest in fulfilling the purpose of personal data processing, which would not be possible without the provision of personal data. The administrator undertakes to process accurate personal data only for the purpose of:

• sending commercial messages (direct marketing).

The administrator undertakes not to process personal data in a way that is incompatible with the above-mentioned purpose.

The administrator undertakes to process personal data only to the extent necessary in relation to the purpose for which they are processed.

The scope of personal data provided based on consent is as follows:

• name and surname
  
• e-mail address
  
• telephone number

(hereinafter referred to as "personal data").

The administrator undertakes to process personal data for a period of 5 years. After this period, the administrator is obliged to dispose of personal data.

The administrator declares that personal data will be made available to the relevant employees of the administrator, who are obliged to maintain confidentiality about this data, as well as about security measures, the disclosure of which would endanger the security of this personal data.

The Administrator declares that personal data will be transferred to the company PS Works s.r.o., with registered office at Jungmannova 1310/10, 697 00 Kyjov, ID: 28306201 (hereinafter referred to as the "web administrator") for the aforementioned purposes, as this company provides website management for the Administrator. The obligation to maintain confidentiality of personal data also applies to the relevant employees of the processor.

The manager also declares that he will not transfer personal data to third countries or any international organization.

There will be no automated decision-making when processing the data subject's personal data.

The administrator hereby informs the data subject that profiling will take place (a form of automated processing of the data subject's personal data consisting in the use of personal data to evaluate some personal aspects related to the given data subject, in particular to analyze or estimate aspects related to personal preferences and interests ). The administrator uses profiling only for the purpose of personalizing business communications for specific data subjects (targeted advertising).

The data subject has the right to revoke the consent given to the processing of personal data at any time without any limitation or damage, either at the address of the controller's headquarters or at the email address mdostal@celpap.cz
Rights of the data subject The administrator informs the data subject of the rights arising from the GDPR regulation, in particular:

• the right to access personal data (the data subject has the right to obtain confirmation from the controller as to whether or not personal data concerning him is being processed - Article 15 of the GDPR)
  
• the right to correction (the data subject has the right to have the administrator correct inaccurate personal data concerning him without undue delay, as well as the right to supplement incomplete personal data - Article 16 of the GDPR)
  
• right to erasure (the data subject has the right to have the administrator delete personal data relating to the data subject without undue delay if one of the reasons listed in Article 17 of the GDPR regulation is given)
  
• the right to restrict processing (the data subject has the right to have the controller restrict processing in the cases set out in Article 18 of the GDPR)
  the right to data portability (the data subject has the right to receive the personal data concerning him/her that he/she has provided to the controller in a structured, commonly used and machine-readable format, and the right to transfer this data to another controller without the controller to whom the personal data was provided , defended, in the cases specified in Article 20 of the GDPR regulation)
  
• the right not to be the subject of any decision based solely on automated processing (the data subject has the right not to be the subject of any decision based solely on automated processing, including profiling, which has legal effects for him or significantly affects him in a similar manner according to Article 22 GDPR)
  the right to file a complaint with the supervisory authority, which is the Office for the Protection of Personal Data, based in Plk. Sochora 727/27, ZIP Code 170 00, Prague 7

The data subject hereby declares that he/she has been properly informed by the administrator about the processing of personal data in accordance with the provisions of Article 13 of the GDPR and that the above personal data are accurate and true and are provided to the administrator voluntarily.
